My alarm module with siren and extension just arrived in the mail, but I have not done the install yet. Since this alarm is compatible with numerous Honda models, it appears that whether or not the extension cable is needed, depends on where the module is mounted on the bike. In other words, is the cable on the module long enough to reach from where the owner wants to mount the module, to the "Option" connector on the bike without using the extension.
On my 2017 CB1100 EX, it looks like I can mount the module to the inner rear wall of the plastic tray that sits over the battery, with black Velcro, and plug directly into the Option connector. With Velcro, I could detach the module from the tray, lay the module aside, and remove the tray if I need to get to or remove the battery. I wont know if that will work until I get to the install. If my plan doesnt work, the other module mount locations may require the extra length of the extension in order to reach the "Option connector. Short answer, the alarm should work as intended plugged directly into the Option accessory plug ( without extension ). I guess since the parts have to be ordered from Japan, I would rather have the extension cable and not need it, rather than need it and not have it. Then too, the extension will open up more options as to where the module can be mounted. I have the 2013-2014 CB1100/A/SA Service Manual. In the General Information section, page 1-38 the location of the "Option" Connector Boot is shown, 2P connector for 2013 model, 4P connector for after 2013 models.

Just a little useful info,anything sold or manufactured for these bikes that was never sold in the U.S. market,no dealer in the U.S. will have any info on the part. I found this out when I started digging for part numbers for the spoke wheel setup on my 2014 Standard.
