Eh dear me. A problem most of us (who care to admit it) have had from time to time.
The HD was good but occasionally got caught out (either not cancelling or cancelling too soon).
I do try and cancel after engaging 2nd gear as a habit - kind of a ritual. Also stab the cancel button occasionally as I'm going along to make sure I haven't forgotten.
I get annoyed when I do forget and for me the solution is taking the innards from a bug zapper and wiring them up to give me a mild electric shock with every flash. That should do it!

H-D pre 04 Turn Signal Modules utilize a combination of time and distance for auto-cancel.
The distance input is from the Vehicle Speed Sensor

04 up use input from the serial data link on a 12 position TSM.

Pre 04 auto-cancel can be defeated by cutting the white/green wire.

Amazing how many H-D riders defeat it!
