Over the last 20 years I've worked with many patients who are in the "self neglect and abuse business", and when things start falling apart it hits them way too hard, not just physically, but psychologically and socially. Destroys their life, kills all the joy, makes everything they ever accomplished meaningless. Winter came and they didn't get out of Dodge.

What I've learned is that early 40s is an age when that "health train" is leaving the station, but if you haven't been too cruel to yourself and make an effort you can still catch it in your 50s. You'll be puffing and panting to catch it, and will probably hurt a lot for a while after you get on it, but make sure you get on it. Pain is a different beast, and it humbles everyone. People often justify destroying their health with having a fat bank account, and from what I've had a misfortune to see in my line of work, not having money is much better than not having health. One can't just throw money at pain and make it go away.

So, my middle aged friends - all aboard, the train is about to leave the station.
